Report A Patient Privacy & Information Security Incident/Breach Before It Becomes A Bigger Problem

Let's "Do the right thing" by protecting our patients' privacy and information security.  That includes knowing who to contact when you are aware of an incident or a possible breach.

No need to worry... help is just a phone call or e-mail away. The key thing to remember is to report an incident or breach before it escalates into something bigger.

If you are aware of a privacy breach, possible incident, or have a concern, contact the HIPAA Program Office at 4-9716 or HPO@bsd.uchicago.edu.

If you suspect an information security incident such as:

• Unauthorized access to the computer network, file system, or sensitive files and data;
  

• The UCMC/BSD network system is being used to scan other network systems;

• The UCMC/BSD network system is being used to store malicious files like computer viruses, worms, and Trojan horses.
