Let's "Do the right thing" by protecting our patients' privacy and information security.  That includes knowing who to contact when you are aware of an incident or a possible breach.

No need to worry... help is just a phone call or e-mail away. The key thing to remember is to report an incident or breach before it escalates into something bigger.

If you are aware of a privacy breach, possible incident, or have a concern, contact the HIPAA Program Office at 4-9716 or HPO@bsd.uchicago.edu.

If you suspect an information security incident such as:

Contact the Chicago Biomedicine Information Services help desk at 2-3456 or send an e-mail to help@bsd.uchicago.edu or Cservices.Srvs@uchospitals.edu for assistance.

Be alert... Take action... Contact help

Call 4-9716 for more details.
