Report A Patient Privacy & Information Security Incident/Breach Before It Becomes A Bigger Problem

If you are aware of a privacy breach, possible incident, or have a concern, contact the HIPAA Program Office (HPO) at 4-9716 or HPO@bsd.uchicago.edu.

If you suspect an information security incident such as:

• Unauthorized access to the computer network, file system, or sensitive files and data;
• The UCMC/BSD network system is being used to scan other network systems;
• The UCMC/BSD network system is being used to store malicious files like computer viruses and worms.
